The Attendant Card

The Attendant Card allows persons with functional limitations to have an attendant traveling with them when they use exo’s regular network. In such a case, upon presentation of the attendant card, the attendant is granted free fare.

• It allows the attendant to use the public transit free of charge.
• It is granted under certain conditions (see the section on conditions).
• It is essentially destined to persons who are not eligible to benefit from exo’s paratransit service, but who still need assistance when using the regular transit network.

1. Fill out and print the application form
2. Have your limitations and need to be accompanied by an attendant certified by an authorized person.
   A section of the application form must be filled out by an authorized health professional*: audiologist, specialized educator or special education technician, occupational therapist, respiratory therapist, general practitioner, medical specialist, neuropsychologist, optometrist, speech therapist, physiotherapist or physiotherapy technician, psychoeducator, psychologist, visual deficiency, orientation or mobility rehabilitation specialist, social worker or social service worker.
3. Send the completed form by mail to the following address: Exo accessibility coordinator, 1001 boulevard Robert-Bourassa, bureau 2600, Montréal, Québec, H3B 4L4.
4. Your application will be examined. You will then receive a letter indicating the next steps to follow (this process can take up to eight weeks).
5. If your application is approved, you will need to go to a service outlet to have your picture taken and get your OPUS card. You must bring:
   • the letter confirming your condition;
   • valid I.D.;
   • payment for the new OPUS card with photo.

If you are not eligible to benefit from paratransit, you can request the OPUS attendant card, which allows you to be accompanied by an attendant when using the regular transit network under the following conditions:

• You must reside on ARTM territory (zones A, B and C) or in off-territory municipalities which have reached an agreement with ARTM in order to provide public transit for their citizens (zone D): Liste des municipalités par zone - Réseau régulier (In French)
• The applicant must be at least 6 years old.
• You must suffer from one or several important and persistent disabilities among the following:
  • auditive
  • cognitive or physical
  • severe learning disability
  • intellectual or ASD
  • motor or organic
  • speech
  • visual
• You must cope with one or several limitations justifying your need to be accompanied when using the regular transit network, namely:
  • difficulty traveling independently
  • difficulty orienting yourself
  • difficulty communicating verbally or otherwise well enough to be understood or understand others
  • difficulty ensuring your own safety
  • difficulty controlling behaviors that may prove harmful to yourself or others.

If you are a user of exo’s paratransit, your paratransit OPUS card already allows you to be accompanied when using the regular network.

• The paratransit eligibility card does not constitute a valid fare: the user must carry valid fare at all times when using public transit.
